TRU PACE is a unique healthcare solution for older adults who struggle with health issues that may jeopardize their ability to remain independent. Most older adults want to age in place, and TRU PACE is an alternative to nursing home care that enables participants to live safely at home with the support of wrap-around care and supportive services.
Helping Seniors Live Safely at Home
TRU PACE is for older adults who meet the guidelines for Enrollment, both functionally and financially. TRU PACE is designed for older adults with limited access to other important services that traditional healthcare does not provide.
Comprehensive Medical Care
Adult Day Center
Day centers with social, medical, and therapeutic activities; meals, recreation, and supervised care.
Behavioral Health
Integrated mental health and emotional support for seniors, focused on well-being, connection, and independence.
Transportation Support
To and from medical appointments or day centers; wheelchair-accessible vehicles.
Rehabilitation Services
Physical, occupational, and speech therapy; recovery and mobility maintenance.
Home Care
Home health aides; assistance with daily activities (bathing, dressing, etc.)
Who Qualifies for TRU PACE
TRU PACE serves older adults from all walks of life. Participants who qualify for both Medicare and Medicaid, TRU PACE covers medically necessary care and services. Our program can enroll individuals who meet the following eligibility requirements:
- At least 55 Years Old
- Living in our TRU PACE service area.
- Certified by the State of Colorado as Needing Nursing-Home-Level Care
- Able to Live Safely in the Community with TRU PACE Services at the time of Enrollment
TRU PACE is a program of TRU Community Care that helps adults 55 and older live safely at home with one coordinated plan. TRU PACE participants receive their care from our expert staff at our center. Our team brings medical care, therapies, social work, home support, and clear communication together under one roof. To make sure you can attend our day health center, we provide necessary transportation to our center for activities and medical appointments.
